For appointments, please call (423) 844-6620, login to your patient portal, or fill out the form below.
If you find that you are unable to keep your scheduled appointment, please give our office staff at least 24 hours advance notice. Otherwise, there will be a $25 no show fee applied.
In the event of an emergency during evening hours or on weekends please call our office at (423) 844-6620. Our answering service will contact the physician “On Call” who is available anytime — day or night — to respond to your emergency needs.

If you are a physician referring patients to Bristol Surgical, please fax records, a copy of the patient’s insurance cards, and our completed referral form.
