Financial Policy...

We participate with most major insurance carriers.  Please make certain that you obtain the necessary referral forms from your primary care physician.

Surgical fees include post-operative services, (usually for 90-days), such as office visits for suture removal and dressing changes.

It is important that you familiarize yourself with the terms and limitations of your insurance contract.  As a service to you, our office staff will complete and submit claim forms to your insurance.  If you have any questions, please call and ask for an Insurance Specialist. 

Please remember – payment is expected at the time service is rendered.  If this is not possible, please notify our Patient Accounts Coordinator, who will work with you to develop a payment plan that is best suited for your budget.

Statements are mailed monthly to patients with an outstanding account balance.  If neither payment nor communication is received within thirty days, the account will be reviewed for external collection activity.
