Our office is open to serve you Monday through Friday, from 9 a.m. to 5 p.m.  For appointments, please call (423) 844-6620.

If you find that you are unable to keep your scheduled appointment, please give our office staff at least 24 hours advance notice.

Please help us maintain accurate records by notifying our office staff of any changes in your address, telephone number or insurance carrier.

Below are forms you may download and fill out to reduce wait time on the initial visit. You may fax, mail or drop them off at our office prior to your appointment.

In order to e-mail these forms to our office you will need to:

1. Print out the form
2. Fill the form out with pen on printed paper
3. Scan the filled out form using a scanner
4. Attach the file to an email and send it to appointments@bristolsurgical.com
